School & Sports Physicals
Physicals are required for all children entering kindergarten, 6th grade, and high school. Older children playing sports will sometimes be required to submit proof of physical exam before allowing participation.
Kindergarten
A complete kindergarten physical requires:
- Complete Physical Examination
- Lead screening
- A screening for tuberculosis
- The parent completes and signs the health history portion of the examination form (state requirement).
Immunizations
- Hepatitis B 3 doses at the appropriate intervals
- DPT/DTaP 4 doses (last one after 4th birthday)
- IPV/OPV 3 doses (last one after 4th birthday)
- Varicella 2 dose after 12 months OR proof of immunity
- MMR 2 doses after 12 months
6th Grade
A complete 6th grade physical requires:
- Complete Physical Examination
- Lead screening
- A Mantoux screening for tuberculosis
- The parent completes and signs the health history portion of the examination form (state requirement).
Additional requirements:
- MUST show proof of Tdap.
- MUST show proof of Meningococcal
- MUST have 3 doses of Hepatitis B vaccine at the appropriate intervals.
- MUST show proof of two doses of Varicella (chicken pox) or proof of immunity.
9th Grade
A complete 9th grade physical requires:
- Complete Physical Examination
- Lead screening is required.
- A Mantoux screening for tuberculosis is required.
- The parent completes and signs the health history portion of the examination form (state requirement).
Additional requirements:
- MUST have 3 doses of Hepatitis B vaccine at the appropriate intervals.
- MUST show proof of two doses of Varicella (chicken pox) or proof of immunity.
- MUST show proof of Tdap.
- MUST show proof of Meningococcal
Just a reminder: If your child is requiring an asthma check or an ADD/ADHD check or follow-up a separate appointment on another day is required.
Per CWC policy, if a child comes to a well visit with an illness, the illness will be addressed and a separate office encounter fee charged and/or well visit may be rescheduled.
